What Is Gout?

Gout is an inflammatory arthritis caused by uric acid crystals depositing in a joint. The first metatarsophalangeal joint (the big-toe joint) is the classic site — podagra — but gout can hit the midfoot, ankle, or Achilles bursa. Over time, untreated gout becomes chronic foot arthritis and can form chalky tophi under the skin.

It is not a bunion flare, and it is not hallux limitus, though a patient can have both.


Symptoms of a Gout Attack

  • Sudden, severe pain in one joint, often the big toe
  • Redness that can extend onto the foot
  • Heat and swelling
  • Pain so intense that socks and sheets are intolerable
  • An attack that peaks in 12–24 hours
  • Residual stiffness for days to weeks after

Fever, spreading redness, or a wound near the joint raises the infection question. We do not guess.


Causes and Triggers

Uric acid rises because the body makes too much or the kidneys clear too little. Attacks are pulled by:

  • Red meat, shellfish, beer, and spirits
  • Dehydration (a New York summer plus a long dinner)
  • Surgery, illness, or a crash diet
  • Certain diuretics and other medications
  • Family history
  • Psoriasis, kidney disease, metabolic syndrome
